Vestige of Flagellation’s first ability generates a replacement effect that applies to the first time an opponent would gain each turn. This is not optional: the first time an opponent would gain each turn, instead you will lose that much and you will create that many Vigor tokens.
There is no priority in between the effects generated from apply the replacement effect (e.g. losing , creating Vigor tokens)
In a multiplayer game, this replacement effect will apply to the first time any opponent among all opponents would gain each turn.
If an opponent’s first life gain effect is replaced (e.g. Poison the Well) and then has another life gain effect later, Vestige of Flagellation can apply to that new life gain event because it would’ve been the first time they actually gain life and can be replaced.

Keyword notes

Bladebreak - Equipment that are fragile and break after being used to defend. If you defend with a card with Blade Break, destroy it when the combat chain closes.

When you defend with a card it remains on the combat chain until the chain closes, therefore you cannot defend with an equipment multiple times on the same combat chain.
When this card is destroyed, it is sent to your graveyard.
The ability becomes a layer on the chain when the combat chain closes.
The Blade Break ability becomes a layer on the chain when the combat chain closes.
The ability goes on the stack when the combat chain closes.
