From the Super Slam release notes

If you control both a Confidence token and a Toughness token, this gets +2. If you control only one of these tokens, you will not get the +2 from its effect.
If you control both of these tokens when this defends, then you lose either one of those tokens, this will lose the +2 gained by controlling both tokens.
If either a Confidence or Toughness token is created during the damage step and causes the if-conditional effect to become active, it will get the +2 from controlling both tokens but the damage will not be recalculated.

Keyword notes

Bladebreak - Equipment that are fragile and break after being used to defend. If you defend with a card with Blade Break, destroy it when the combat chain closes.

When you defend with a card it remains on the combat chain until the chain closes, therefore you cannot defend with an equipment multiple times on the same combat chain.
When this card is destroyed, it is sent to your graveyard.
The ability becomes a layer on the chain when the combat chain closes.
The Blade Break ability becomes a layer on the chain when the combat chain closes.
The ability goes on the stack when the combat chain closes.
